Cancellation Form Request

1. Did you try a Membership Hold?

Memberships holds are $30/month and while on hold you can still attend 2 classes per month.

Membership rates are subject to change and you are currently locked in at your current rate. If you were to cancel and rejoin at a future date you would be subject to new pricing, and the membership reactivation fee. But with the Membership Hold, you keep your current membership rate locked in for good.

2. 30-Day Notice

If you wish to proceed with your cancellation. This form will serve as your 30-day cancellation notice. Your membership will be cancelled 30 days from the date submission of this form.

3. All payments are non-refundable

Please note, scheduled membership renewal payments within this 30-day period will be processed as scheduled. We are unable to provide refunds.
